Replace Radio Buttons with Stars?

My client would like stars on his survey form, instead of the radio buttons I have
    for him.

Here is what I mean... here is the JotForm I made for the client (note the radio buttons for ranking):

Here is the survery form that has stars on it (for ranking the business from "Poor" to "Excellent") like the client wants:

In other words: Can I replace the radio buttons I have on my JotForm, with some kind of selection stars like on
the other sample survey I showed you?

    Thank you

We can do something similar with the Star Rating tool:

It is possible to inject a custom CSS to add the POOR and EXCELLENT words:

    margin-right: 10px;
    margin-left: -25px;

#cid_1 is the ID of star input from my form, so you will need to use your ID from the form source.

If you need any further assistance, please let us know.

Thank you very much, Mike_T -- It's almost working now...

I wasn't clear on the ID thing, so I cloned your form and built a new survey from it, here:

But when I fill out the fields, the cursor turns into a red circle with a line through it.
Then when I click on the page (outside of the form) to get rid of that cursor, I am brought to our website! Even if I don't fill out the form, and just click on the page, it stills goes to

If I could just get rid of my cursor turning into a circle with a red line through it, I would be happy.
I'm not worried about the rest.

I'm using IE -- but it seems to do the same thing in Chrome.

    Thank you.

I've  not been able to replicate the cursor issue but it sounds like it may be a local setting on your computer:

Do let us know if the video helps to resolve the problem.

Hello abajan -- Thank you for your reply...

the main problem of concern right now is that, in the event that someone begins to fill out
survey form, then wants to contact us usung the other contact form (at right) they cannot
click on the fields to type (at least I can't when I try -- cursor turns into that red circle/line).

Here is a screenshot of what I mean:

    Please advise. Thank you.

Have you tried the star or scale rating fields under the survey tools?

Please give them a go and let us know if they need further tweaking on your form.

    Hello TitusN ...

Not clear on how that addresses my most recent request...
Are you saying that doing as you suggest will solve the cursor/typing issue?

I checked your website but I could not find the dropdown you describe on this screenshot . Have you perhaps modified the form already?

This is actually a different issue from the original subject you had raised about Radio button replaced with Stars...

I highly suggest that you create a new separate thread on this so we can dedicate the appropriate support on it. Use this link to do this

Our colleague TitusN was refering to the original requests/inquiry of this thread.
Can you please also confirm if that issue was resolved already?

    Hello Jonathan,

I should have menioned I'm usung IE 9 -- and that you must first begin filling out the survey form, then click on the email form, to duplicate the field issue that I sent you a screenshot of:

Unfortunately, I wasn't able to reproduce the issue that you are referring using IE browser.

Have you tried already clearing your browser cache and cookies? If not, please give a try.
