    Thank you for contacting us. Can you share your website URL so we can do some test. I can see that you the width of the form is set smaller what we need to see is the website URL how the form will be adjusted. In the meantime can you try embeding the form using an iframe then adjust the width of the frame according the size that fits your site.

    You can change the one that is highligthed in red.

    <iframe id="JotFormIFrame" onload="window.parent.scrollTo(0,0)" allowtransparency="true" src="//form.jotform.co/form/40073999991875" frameborder="0" style="width:100%; height:501px; border:none;" scrolling="no"></iframe>

    <script type="text/javascript">window.handleIFrameMessage = function(e) {var args = e.data.split(":");var iframe = document.getElementById("JotFormIFrame");if (!iframe)return;switch (args[0]) {case "scrollIntoView":iframe.scrollIntoView();break;case "setHeight":iframe.style.height = args[1] + "px";break;}};if (window.addEventListener) {window.addEventListener("message", handleIFrameMessage, false);} else if (window.attachEvent) {window.attachEvent("onmessage", handleIFrameMessage);}</script>

    Let us know if this works for you otherwise we need to see the actual website where the form is embeded to see it in action.
