Form Keeps Scrolling Down

Hello,

When I try to fill out the Patient History From on:

1. The form keeps scrolling down and will not stay in place.
2. Also, when I get to the point where it asks if you are married, single, etc. it won't let me check off the box.
3. Lastly, all of the forms lost their designs and are black and white in a Times font.

I cleared my cache and the scrolling stopped, but I'm still having the same problems. Please see screen shot.

I suspect that there might be script conflicts between the form and the website where it is embedded.

Can you please embed the form via the iFrame method?
