COVID Responder Account Application Status.

Hello,

I am writing to inquire about the status of the COVID-19 free account application that was recently submitted on behalf of the San Mateo County Health Foundation from this account. Our Foundation is hoping to begin using a HIPPA compliant JotForm to support our patient assistance program which directly helps patients who have been affected by COVID-19. We hope to begin building the form out in the next few days but are waiting to recieve a response regarding our application.
