How to set up a notes section for multiple inputs?

    Good day all.

    My form has a notes section with two fields per line, "Time" and "Notes" (image below). What is the best way to set this up? I tried using the Configurable List Widget with Time and Text Area but could not map the fields to the PDF. Also, when I previewed the form those fields did not display.

    Any suggestions?

    It's not possible to map the configurable list widget. You'll need to create a separate fields for time and long text field.

    I can see that you have already added that and it's mapping the data correctly.

    It's a great widget if I don't need it mapped to the PDF or can do it later in another app. It would be outstanding if it did.

    It is possible not to map the fields to your original PDF.

    The data will still show in the table submission.

    You can then manually download the submission as PDF in the table submission.
