Form shows date of birth instead of name

Hi, I one of our form is showing date of birth in place of Student or parent name. Plz find the attached file. How we can change this to name of students or parents…?

As I understand, you want to change the submission header on the mobile submissions page. Here is a step by step guide for you to follow,

1- Go to the Submissions page.

2- Click one of your submissions.

3- Click the 3-dot menu at the top right.

4- Click the View Settings.

5- Select field for the submission header.
