How Can I Use a Custom Domain Name?

    Hello Team,

    How can I change the link name? it always shows what I believe is my username or email address = jbpadilla.maxicare, this shouldnt be included to the link. Let me know where is the settings to remove or can you just replace it with maxicare only

    Hello,

    Thank you for contacting JotForm Support.

    You can add your own form name at the end of the URL along with your JotForm Username. For this, you can apply the following steps:

    1. Edit your form.
    2. Go to the Publish tab of the form builder.
    3. To start creating a custom URL of your form, click the pencil icon.
    4. Upon clicking the pencil icon, you will be allowed to type in a custom URL of your form. Once you're done, click the Apply button.
    5. Your custom URL is now ready, copy the link, or open it in a new tab.

    But the custom form domain is only available on our Enterprise system. You may visit the following links for further information:
