Cannot cancel never booked

Hello I used this software once and now it’s randomly booking me into this place again. There’s no cancel button.. extremely poor software.

You can delete the submission related to that appointment from your form submission page - https://www.jotform.com/help/377-How-to-Delete-Form-Submission-Data 

This will reinstate the appointment slot on the field.

I don’t have access to this stuff? I’m a customer of the place. I have no way to cancel the appointment that I never made...
