In the form below I have created conditions for frames, to show the questions for that frame style. In another form it is working for the first style but missing temple style for the others. this form was cloned from the same one I tried to deleting the action and add it back and now not ony is it not showing temple style but also showing options for the next lens style.

Is that the way you wish the form works?
Your Temple Style field is not displying because there is a conflict between the conditions you have created.
Since you have three separated conditions to show Temple field, the system do not know which condition to obey , so that is the conflict. So, you need to delete that Action from the three Conditions.
Then, you need to create a separated conditions that will show Temple Style field if any of the Frame style options is chosen:
After that, to prevent the other lents styles from appearing I recommend you to use Form collapse to hide/show the multiple fields as instructed in this guide: How to Hide or Show Multiple Fields at Once Based On a Single Conditional Entry
Then create the conditions to show the form collapses, not the fields.

In my example form in the previous message I did not fix the problem of the other lents styles fields appearing even if that style has not been selected. But I cloned my form and fixed it already. So, here it is: http://jotformpro.com/form/40094999057972

Thanks! It would be helpful to see the conditions you used. I am not really understanding how to collapse. I had thought that was what I needed to do but wasn't clear on how to do it.
