    After I downloaded the form, the format is not the same as the online version, they all moved..... and not aligned on the same line. How to resolve it?

    Are you referring to the form submission data sent to you in PDF?

    I have cloned your form, which is the only form in your account, and downloaded my submission data as a PDF file.

    Here is the result:

    As you can see that the format looks fine.

    It would be helpful for us to understand your issue if you could share your screenshots.

    I still cannot see any misalignment as you described.

    Yes, when you see the pdf on the screen, it looks perfect. But I am talking about once you downloaded it as a pdf and open that file, it is all misaligned. Please advise. Thanks.

    What form are you referring to? Also, what software are you using to view the downloaded PDF file?

    Please also share a screenshot of the "misaligned" PDF so we can better understand you.

    Hi!

    The one that you adjusted for me, it looks a bit cleaner, but it is still not what I want.

    The fillable bubble should be on the left hand side of each item, but on the pdf, it was shown above the items. See attached.

    I do know how to adjust via pdf...

    Another question, can we do e-signature?

    Unfortunately, it's not possible to change the position of the "bubble" to be on the left hand side of each field.
