Issue with link

i have created a new link and the link doesn't work i dont know why since i renew my serviced im facing the same issue again and again ?

I was using Google Chrome on Windows 10 when I tested your form.

Do we have to submit the form to see the issue? If that is the case, with your permission, we would like to make test submissions on your form.
