Special Form requirement

Hello -- I represent a global nonprofit (avp.international) that offers workshops in over 50 countries. We are looking for a survey system where participants in a workshop anywhere in the world can complete a survey (or evaluation) and the results can be shared directly and simultaneously to both the local organizers for their information and to our central database for long-term research. Can Jotform help us with this project?

I have been using Jotform for a simple data collection form, but this project is more complicated:

    • We would need (for example) 20 countries each entering workshop evaluations (mostly text responses) on their own form (to accommodate different languages). The form for each country will have exactly the same fields and contain an identifier for their country.
    • We want each country to be able to access reports for their own submissions.
    • We also want all submissions sent to our central database automatically. Jotform has a bewildering number of Integrations, but I saw "Combine Submissions" that might serve that purpose.
    • Any other solutions you might advise?

    Hi,

Thank you for providing more information on what you want to create.

    You can achieve this by:

    • Creating 20 forms for each country.
    • Using iFrame embed form.
    • Using show or hide condition.

Here's a simple demo form with only 3 countries option: https://form.jotform.com/211636281662051

The form will appear based on the answer of the user to the form. This workaround will separate the submissions per country. You can also create a Grid Listing Report for each form (or country) without confusion.

Here are the guides that will help you achieve the demo form:

    1 - Create a form: https://www.jotform.com/help/2-how-to-create-your-first-web-form

2 - Widget where you will add the form links to the main form: https://www.jotform.com/help/148-getting-the-form-iframe-code

3 - The condition to show or hide the embedded form: https://www.jotform.com/help/316-how-to-show-or-hide-fields-base-on-users-answer

4 - Here's a guide on how to create a Grid Listing Report for each country: https://www.jotform.com/help/105-how-to-create-a-grid-listing-report

    Would that work for you?

Thanks, that is helpful and looks close.

The Grid listing would be fine for the central database where ALL submissions need to be listed. However, I don't see a way to filter the submissions. The Grid Listing Guide mentions a search feature, but when I created a Grid Listing from one of my forms there was no ability to search.

Actually, we need to limit the Grid seen by each Country to ONLY submissions from their own country, but let them search (filter) their submissions based on workshop date (which would be one of the form fields).

    Hello! 👋

It sounds like Tables may work better for you. I've created a new demo form here based off my colleagues example to include three countries. USA, UK, and CA. When you complete a form submission it goes to this Table here. Feel free to create form submissions and test this yourself.

The submission data comes into one of the three tabs based on the users dropdown selection on the demo form. The guide on how to do this is here. The Table I shared at the top contains all three Countries but you can also choose to just share a specific Countries Table since every form gets its own Table, example for UK is here.

We have a guide here that goes into detail on how to share out your Tables. You can also filter the data in tables.

Or you can create tabs that contain custom reports all in the same Table based on each country.

More information on this feature is here. We also have a help section here dedicated to JotForm Tables that can help you get started with the platform. I hope this helps! If you have any other questions or need any alternative solutions, please let us know, we'd be happy to help!

    Thanks!
