Add Multiple Instances of a Group of Fields

    Hello,

    I'm trying to create a form to collect information from parents on their children who will be participating in the kid's ministry at our church. We need to know the name, date of birth, allergies, and learning needs of each of their children (I've attached a screenshot of the form as a Word doc for reference).

    Ideally, I'd like for them to be able to add as many or as few children as they have in their family, and display the same group of fields one time for each child. What is the best way to accomplish this?

    Right now the best ideas I have are to use an input table (although it's pretty cramped since all the fields have to fit in a single horizontal row) or to use conditional logic (although I'd have to create probably at least 5 instances of each field and then apply conditional logic to every single one of them based on how many children the parent says they have).

    Is there any simple way to have a parent click some kind of "Add Child" button that would then display the group of fields we need to collect for that child?

    This can add another layer of the same questions to the form. You can add as many as you want or you can limit it up to 10 or more. Here's a test form that you can check: https://form.jotform.com/211666966177065
