    I need help

I have 2 lines at the bottom of my form that will not place where requested.  The 2nd to last line should have 2 entries and the last line should have 3.  but for some reason, 2 of the entries from the last line keep jumping up to the previous line once I copy the script to my site

    Hi Madison

I believe this problem may be caused by the width of the embedded version of the form but a definitive answer can only be given if I am able to view the form in context. So, could you please provide the URL of the web page in which the form is embedded?

    Thanks.

Unfortunately, that page is not published yet. I’m still building it in publisher. But, all I’m doing is inserting the script into an htlm code fragment block. The width is adapting to the width in the script.
