Not seeing submitted data

    I have a form "Eye Exam Request" and two people have completed the form via our website (a link not embedded). I see they completed the form but when I go to the inbox I do not see the data. It says "Submission 36" and "Submission 37"

    Since I will not be able to view your form submissions due to HIPAA restrictions, I have created a cloned version of the form you are referring to in my account instead. Here's what I see on my end when I view a test submission on the Inbox page.


    Perhaps, in a form of a screenshot, you will be able to show us what you are seeing on your end.


    Also, may we ask that you check what is set under the Header Settings of your form's Inbox page, please?
