Responses are not passing from one form to next for only the first 2 questions

Hello, for some reason, the first two responses on the parent form are not passing on to the second form.

I have no idea why.

I've also tried to use the new Approval Flow feature but I don't think it works for what we need in this situation.

The issue is caused because you are using incorrect unique field name in the URL for the first two fields. The correct field name should be whichLocation and howMany.

Please use the following URL and that should solve your problem:
