There is one form on your account which I can view it fine.
Please logout from your account, clear your browsers cache and cookies and relogin. The form should be on your My forms tab.
Let us know if you still have issues after that.
There were 5+ forms in there last week.
I logged out, cleared my cache, but still cannot see the other forms. I did change the email address on file for this account but not the user name, could this be contributing to the missing forms?
Upon making a search using your IP address I found that you have lots of free registered accounts, more than 10. So, it is possible that you created those forms in an account that was not AngeliqueBabson.
Could you please share with us the URL of those forms if possible? That will help us to know if your form are in your account or not. I checked the folders of your account, but I was not able to find any form other than the one my colleague found.

Ok, after making a search using the form IDs I found that those three forms are not in the account AngeliqueBabson, but in the paid account ts_form_admin, that is why you cannot find them in MY FORMS page of AngeliqueBabson.
We can move/transfer them to your account, however, you need to make the request while logged in from ts_form_admin as that is the account where the forms currently are. Or if you can contact the account owner, just request him to ask us to move your form to the desired account.
