We searched the database for this form however it seems that this can
no longer be found. We apologize for this issue and inconvenience.
I can see that you have created another form on your account similar to what
is currently displayed on your site. Please update your website with the
new code of the duplicate form so that moving forward new
data submitted is saved and email notification sent.
For the meantime I have opened a request for our admins to retrieve
the submissions on the old form.
