Received help last week with this same issue, and went through the steps and completed, but still are receiving notfication and notification 3. Please advise on how to fix this. See attachment.

This is because in your form you have created 3 notifications,
So you must delete the extra ones , just open the Compose Email wizard and click on "Delete Email"
