Can two signature be placed on the same form at separate times by two different individuals?

Once a client submits a health screening form using a HIPAA compliant Jotform, the health provider will need to review the information with the patient. The health care provider will need to sign off on the health screening form previously filled out by the patient. Is it possible for the health care provider to sign off on the same form using a tablet?
