Questions not showing up on form

Questions on live form not showing up. For example the first one is my form when editing but once IM done it looks like the one below (not showing name email theme etc)

Hi there - It seems you accidentally hidden the labels form the form's advanced design settings. Please set it back to Normal Labels or Bold Labels to have it shown in the form again.
