Printing an application form

    When I print out the application forms I need the parent and participants name to show up on the top page and it's not there.

    I have tried to edit the inbox settings.

    Please help!

    Hi,

    thank you for reaching out to us. If I understand correctly, you are printing the submissions from the Inbox page and would like to display the names on the top.

    I'm not sure which form you're facing this issue with, so I've checked the Inbox on your form https://www.jotform.com/form/211043585870354

    If you would like to display the names as headers on the top of the page, please click "More" and then click "Customize PDF":

    You will be able to add form fields to the page anywhere you like by simply pulling them to the page, and then print using that layout:
