How to conditionally send an email

How can I create either a text field or a a Radio Button field that is hidden, and populated by selection of some multichoice fields?

I have set up a multichoice field (#9): with 8 options.

I would like to send an email to an admin, if any of the first 3 options are checked.

I am unsure how to configure this. My test at present doesn't work.

I apologize for any inconvenience. I have created a clone of the form that you linked to. I see that you had disabled the condition that was supposed to send the email if the right checkbox was selected. I have enabled this condition and I received the Notification 1 email when I selected one of the first 3 checkbox options, when I do not select one, I am not sent the email.

Please ensure that your condition is enabled and try testing the form again. Do let us know if you're still not seeing the email show up in your inbox.
