Help creating order form

I was wondering if there was an order form template that would fit my needs. I have about one or two hundred over the counter supplements that I need to add on an order form. I would like clients to be able to search by supplement manufacturer or name and I don't want hundreds of items to be listed on my form all at once, I would like them to be hidden if possible until the client selects a manufacturer or types in a product name.

You'll need the show and hide condition so that not all supplements will be displayed at once. For example, you have a dropdown field that have these 200 supplements listed, you then create a condition to show other fields when certain supplement is selected.
