My request for HIPPA compliance free access

I filled out a form to receive free HIPPA compliant access on Jotform several days ago for my sole practitioner Acupuncture practice The Integrated Body of Atlanta and haven't heard back yet. I would like an update so that I may ( hopefully) continue and be able to use it for my Personal Health Info. to new incoming patients. Please let me know the status and what I need to do to.
