Hi,
Is it possible to use Jotform to create scorecard questions (yes/no) based questions? The goal of this is to send visual results to the applicants with our evaluation of their situation?

Thank you, but how can I get the customer to view the score at the end or send them an email with the score?

Hello,
You can create a scorecard form at JotForm.
While you creating a scorecard plead follow the steps below;
Go to the Basic tab into the Form Element and click to the “Single Choice” field to add it to your form.
Add your questions in order as in the example below and score their answers
To ad score use the Options under the Single Choice properties field.
Turn on the Use Calculation Value button and define your values to be used in calculations
Just be sure to have the calculation values for each field in the form
You can hide the calculation widget if you want it to be hidden in the users.
In the total score field you can calculate the score.
At the end you can add the total score in your Thank You page by the Thank You Page settings.
If you want to send details about the form you can create an Autoresponder Email in the Settings tab.

Hello Jotform team,
Thank you for the information. I have followed these instructions, however, during the process, I have faced a couple of issues as shown in the screenshots:
1- No calculations
All fields have values:
2- Unable to save required field (Recipient Email)

Hi there,
The Form calculation widget in itself calculates all variables in it so there is no need to place an equal sign into it:
As for the email recipient, I checked the form https://www.jotform.com/form/211752640338049, and it was able to detect the email field in the form:
Simply select that and hit save and you are good to go.
