I'm not sure where to post this question about the SalesForce app. Am posting it here to see how to get an answer:
We are not salesforce users, but are pretty good at mapping fields. However, the "add new field" button is not showing all the fields in SalesForce, especially custom fields added by the SalesForce owner. Can you tell us why all fields are not showing up? Also, there is no "Company" field on this particular form. How can we get to the "next" screen when "company" is blank?

First off, the fields must be created on your form in the form builder. Second, the field types should matched on both your form and the SalesForce form, e.g. if it's a textbox field from SalesForce, it should be textbox field on your Jotform form, if it's checkbox then create a checkbox on your form.
Regarding on the company field, I'm sure it is a textbox field so make sure you have company textbox field on your Jotform form. As of checking, I can't find a company field on your form (40135517741146).
