1) ideally inserted into a google workspace form. Pricing on such?

2) if not, what is your price for smallest plan with HIPAA?

1. All our e-Sign widgets are HIPAA compliant. Here is the complete list: how-to-add-an-e-signature-to-your-form. I'll still ask you to get in touch with your legal advisor regarding the e-Sign widget you choose.

2. These widgets will work only with JotForms. Also, we can assist you with your queries regarding our platform only.

3. For HIPAA compliance, you need at least SILVER membership as specified on our pricing page.
