I have one form and if users answer yes to a particular question then they will need to fill out another form. Is there a way to send them to that new form automatically if they answer "yes" and if it is answered "no" then skip that form? I would like all the information submitted together.

You can use our Conditional Logic (Change Thank You URL after submission). So, if someone will choose "yes" from your settings, automatically redirect him/her to another form after he/she click the submit button.
