I was able to receive the response via email. So in behalf of Heidi, I'm posting it here.  I will also set this thread to private as there are some sensitive information in the submissions:

    Hi,

You can have as many of the new emails as will help. Theyre coming through
and more came today, but no new ones have shown up on they site in the list
to look at or download since February...

Here are the new ones as attachments... The last one, from Jennifer Nichols
is the last that has shown up in the reports...
Thanks for checking. The other forms are working fine. I was just worried
that weve lost some apps because I didnt realize this and log all the
    emails well all the time.

    T-shirt size: S, M, L L
How long have you ridden, what's your personal riding background? I
started riding everyday when I retired from my career job in 2005. The ranch
where we board our horses found out I was pretty handy, so I worked my way
into an informal ranch hand job, and six years later, that has evolved into
    a regular gig.

Now, in addition to doing lots of ranch maintenance chores, I do a lot of
exercise riding for clients, lots of different breeds of horses, levels of
    ability and training.
What do you envision a show being about with your horse? What are you
working on? Chaco is a mustang who has matured into a wonderful horse.
We've grown together and now, we make a pretty good team. More than any
other horse, he has made me a better horseman. He has taught me how to think
like a horse, and I can pass on what he's taught me to other horses. But
he'll always be a wild horse and there's a side of him that's somewhat
    mysterious.

While we are fairly advanced in our arena skills, Chaco is still quite
nervous about things unknown. His wild horse "flight instinct" takes over
whenever he's in a situation he fears. So he needs more sensitivity
training, especially for things around his legs and feet.

I guess I'd like your show to show what a mustang can do -they are
incredible horses - but only in the hands of a person who has the knowledge,
experience and patience to work with them. They are not for beginners or
people who think they are "saving the mustangs" and have no practical horse
    experience.

Tell us about your horse's training and behavior--how much training has he
had? Does he always do what you ask? Is he pushy? Accept a saddle? Buck?
Chaco is a mustang - from a WY wild horse herd, captured by the BLM when he
was about 2 1/2 years old, held and "trained" by the WHIP program in Canyon
    City.

We adopted him March, '05 when he was roughly 5. He was considered a "saddle
trained" horse with 90 days of "training".

We quickly discovered that this horse was still very wild and green,
unsuitable for a novice rider and potentially dangerous.

After several years of uncertainty of what to do with this horse, I took him
over. Since the spring of '10, he has been 'my' horse.

We ride ACTHA, VRH clinics, cow clinics, and more. Chaco is great horse with
    an exceptional mind.

How long have you ridden, what's your personal riding background? I have
roots in dressage dating back to the 70's. Then a long break. I came back to

great trail horse. I am interested in learning more about current techniques
    and equipment.
What do you envision a show being about with your horse? What are you
working on? As a returning, older rider, i represent a growing
demographic. While I do not have fear issues, I very much care about
personal safety. I am working on clear communication with my horse and
getting him to be light in my hands. I could use ideas and help in this
    regard.

Rhyker is an agreeable boy, with a very spoiled background. he belonged to a
fearful person who did not ask much of him. I have worked hard to gain his

has been riding for 6 years, and has been involved with 4-H since she got
her first horse. She has had Rooster for a year, he is 4 now. He was trained
to be a reiner, his sire was an award-winning reiner. Rachel is working on
Barrel Racing with Rooster, keeping the reining foundation fresh for a nice
zippy turn around the barrels! She also has some English training, and has
    had some Hunter Jumper lessons.
What do you envision a show being about with your horse? What are you
working on? Rooster needs to be finished on the barrels. We just began
putting speed on them this spring, we thought he was too young at 3 for
vigorous barrel work. He has problems with his bit, we are trying to find
the correct bit for him. He is always bitting and playing with the bit. He
needs to learn how to stand quiet, be more responsive to leg pressure,
become better at turning on his hind quarters.
Tell us about your horse's training and behavior--how much training has he
had? Does he always do what you ask? Is he pushy? Accept a saddle? Buck?
Rooster had a few months of reining training before being auctioned at the
National Western Stock Show in 2010. A barrel racer bought him and began
working him on barrels. We bought him from that barrel racer in May.

Rachel has done a lot of Parelli ground work with him. She has even ridden
him bridleless and can liberty lounge him.

turned 50 yo, I decided to fulfill a lifelong dream, and learn how to ride.
I'm taking lessons, going on trail rides, started ACTHA rides last year, and
just love learning to be a better rider for my horse.
I never rode as a child, but had a plastic horse collection, even slept w/
    the plastic horses!
What do you envision a show being about with your horse? What are you
working on? Cantering. My buckskin mare never had much training for her
first 5 yrs., she was a pack horse in western Colo., and she and I are
trying to teach each other the correct cues, seat, and she is just way fast.
We work on slowing down every day we ride, now I'm not cantering at all,
because we're trying to slow down at the trot.
Tell us about your horse's training and behavior--how much training has he

Katrina was named after the hurricane, because she is so fast. She is so
friendly w/ people, doesn't buck, we even put grandkids on her, but never go
more than a walk w/ the kids.

How long have you ridden, what's your personal riding background? As a
child I rode on a ranch playing on the trail and in the cow pastures with
the gentle child broke horses. As a teenager I rode in the Westernairs drill
team and liberty jumping and a few small gymcama's, and did lots of trail
riding on my quarter horse. found boys, school and at 20 yrs lost interest.
Started up again in 2003, 2004 with a horse that was way too much for me,
got hurt lost confidence, but stuck with it, have a great horse, almost sold
him to you after being hurt again, but stayed with it. I have improved but
have had some spooking issues when a one reins stops was needed, or bucking
in front of a dressage show judge, and using too much of the wrong leg and
having Buck go out from underneath me. I am starting to canter indoors with
some confidence. but have not tried the flying lead changes. Buck and I have
a 60% Balanced stop, roll backs are slow but correct, spins are slow but
correct mo! st of the time. leg yeilds, side passes are pretty good. Have
been training with Rusty Hall, and have come a long way, but have so much
more to learn. Am curently playing with western Dressage, and recieved a
ribbon at the gamblers choice (Timed Trail obsticles) in Salidaa couple of
years ago. And would someday love to do some Stock horse, or Ranch
    Versatility.
What do you envision a show being about with your horse? What are you
working on? Riding and personal Confidence. At 230 pounds and 5'4" I can
love and ride horses just like anyone else with or without a handicap. I
just need to be balanced in the saddle, have good tack, use a three step
mounting block, and be a fair leader to my horse. I would love for people to
awaken to the thought that just becuase the body isn't the norm for the
sport, that desire, and love is everything, maybe more people would be
interested in horses. And I really think if you ask Buck he would say my
weight hasn't been an issue to him. He just wants respect like the rest of
    us.

The picture of Buck is 2 years old, and is not a good picture on him. I have
recent video of us but do not know how to download it to anyone. The picture
of me is just a head shot - dosen't show the 230 pound butt. I am not very
photogenic. There is a picture of us on facebook riding in the Christmas
parade in costum, and playing soccer with the horses one day.
Tell us about your horse's training and behavior--how much training has he
had? Does he always do what you ask? Is he pushy? Accept a saddle? Buck?
The Best person to talk to about that would be Rusty Hall. He has been
working with Buck and I for almost 4 years now.

I ride about 4 days a week Saturday and Sunday I ride in Clinic at the barn
where Buck is. On Tuesdays I ride a private lesson. and then one or two
other days a week I just work in the arena on what needs to be worked on.

Buck was a 4 year old Roping horse that won a little money. He is a Two Eyed
Jack breed gelding, Dun with Black points 15.1 hands about 1150 pounds. He
is well musceled, and very powerful, sometimes his power scares me a little
    and I hold him back.

Buck, has his issues, Respect has been a constant with him. sometimes
depending on his mood if I give him an inch he will take a mile and run with
it. He is courous to the point where he looses attention easily, especially
if I stop and talk to someone. On long lines I have had him do great roll
backs, not want to move at all, and he has not recently but in the past
charged at me in the round pen. So Respect is a big constant for us.

How long have you ridden, what's your personal riding background? I've
been riding for approximately 25 years. I grew up doing hunter/jumpers with
some time spent in western pleasure and equitation in addition to
competitive trail riding in New England. In college I trained horses for a
hippotherapy stable with Ann Street-Joslin in Fort Collins, CO. I've been
eventing since '97 (schooled through intermediate), and currently I own and
operate Bit of Honey Training LLC in Fort Collins, CO. I start young horses
under saddle (some english, some western with cattle work, all trail ride as
well) and compete my own Thoroughbreds in dressage and eventing. I teach
dressage, jumping, western, and trail riding to my students.
What do you envision a show being about with your horse? What are you
working on? Bovineaphobia! My eventing horse is bold over fences, brave
on the trail, and balanced in the dressage arena, but seeing or smelling
even one cow absolutely undoes him! Living and competing in Colorado and
Wyoming, our show grounds often host cattle events as well and it has been
the downfall of our competitions to have him smell or see a cow and lose his
mind. He bolts and doesn't stop running until he feels like he is a safe
distance away from the horse-eating-machine. He is fine with dogs, cats,
alpacas, llamas, goats, sheep... just terrified of cows.
Tell us about your horse's training and behavior--how much training has he
had? Does he always do what you ask? Is he pushy? Accept a saddle? Buck?
He has competed through first level dressage and is schooling second level
movements, novice level eventing, show jumping up to 3'3", and has done 15
mile trail rides. He is very willing, friendly, obedient, and easy to work
with in hand and under saddle except for the fear of cattle.

How long have you ridden, what's your personal riding background?
Lifetime of ranch/trail riding. Started showing 3 years ago - this year
starting to ride Reined Cow Horse & reining competitively - entry level
    showing.

After my accident several years ago, I'm gradually working into competition
and continually working on balance, sitting back and continually working on
    becoming a more confident rider.

I'll Be Bud Lite was 2010 APHA Working Cow Horse Honor Roll Champion in both
Jr and Open competition. He was my main show horse this year, but he needs a
break and thankfully, I'm blessed with great show horses, so he's home on
    break!
What do you envision a show being about with your horse? What are you
working on? Still working on riding confidence, balance and remaining in
a good zone as my competition gets faster (reining & cowhorse). I did
succeed in western pleasure, trail, horsemanship and showmanship and was top
10 RMPHA. However, now that we run a registered angus cattle operation, I
need to kick it up a notch whether I'm checking cattle or sorting pairs on
    the ranch or showing.

If nothing else, my story should be an inspiration to Dream it, TRY it, DO
    IT!

He is a gorgeous mover and is photogenic!
Tell us about your horse's training and behavior--how much training has he

My horse is a seasoned show horse. I get along with him great; however, I'm
not as confident as I should be when he is fresh. He's been professionally
trained for 3 years and has had a very successful show career and we have
had a successful show season so far this year, but just brought him home
    before he get's fried...

    T-shirt size: S, M, L small
How long have you ridden, what's your personal riding background? I
started riding when I was 58 following 58 years of being petrified. I was
able to conquor that fear with a little help from a clinic with Julie on
fear management. I had a rocky who I rode for two years who spooked and
tossed me two and a half years ago. I got a fractured back from that spook
and a new horse better suited, we thought, for my needs. I've ridden Bentley
now nearly two years with relatively good success. My fear continues to be
an issue, however and I'm not confident enough yet to get out on the trail
though we are both better. I take as many lessons as I can afford and as
many clinics as I can manage. My issues seem to be around confidence and
when I get tense, I tend to get more ineffective in my use of my aids.
What do you envision a show being about with your horse? What are you
working on? I'd like to work on using my aids more effectively as well as
building my confidence so that, when Bentley spooks, I'm able to manage it
in a helpful way. Right now, I'm working on making my aids more effective.
The other thing I'm working on is getting out of his mouth more and still
having effective aids. I'm thinking that if I'm more effective and he
listens better, when we're out on the trail, I'll be more confident and able
to use my "go to" choices when he spooks.
Tell us about your horse's training and behavior--how much training has he
had? Does he always do what you ask? Is he pushy? Accept a saddle? Buck?
I bought Bentley when he was 6. He'd reportedly been a much loved guy whose
rider had an accident and couldn't ride him for a year before she sold him.
She kept him as a lawn ornament for a year. Someone put 90 days on him
before we bought him. He and I have had lessons weekly for about a year and
a half total. He doesn't always listen to my aids the first or second time
but eventually does do what I ask. He's rock solid on trail issues and very
careful about where he puts his feet. On the other hand, he can spook in the
arena if someone comes in with a yellow coat on. He's good about the saddle
and bridle. No bucking. He can be pushy if he wants you to scratch him but
also responds readily to a request to back away. Spooks aren't bad now. They
used to be a spin and canter off. Now, he startles and is resistant to
    moving on.

Please describe your horse riding background... I've been riding since I
was 10 years old... Began with English (dressage & jumping), and progressed
to western. For the past 10 years, I've been riding Rocky Mountain horses,
and have done lots with the gaiting work.
What topic would you most like help with in your horse's training? Angel
is a 7 year old mare this year. I started her under saddle as a 3 year old,
and she was pretty nice out on the trail. She had quite a few years off to
have a foal. I began working with her again last year, and have done quite a
bit on the ground with her. In the saddle, she's done much better in the
arena. When I try and take her out on the trail, she becomes quite spooky
and is somewhat herd bound. I'd like to have some help getting her over this
    issue.
How many days of professional training has your horse had? None. All work
    has been done by myself.

**Note: I currently don't have a video of myself and Angel at the moment as
    it's been too cold.

    Hello Heidi,

It was just now that I noticed that your form on the website is under v2.jotform.com.  I'm not sure if you have the same form id in v2. But to receive your submissions under the curren version page, please do change the code in your page and set it to http://www.jotform.com/ both in your iframe tag:

    <iframe scrolling="no" frameborder="0" style="width:520px; height:2143px; border:none;" src="http://v2.jotform.com/form/82040150217">

    and the form submit tag:

    <form name="q_form_82040150217" method="post" action="http://v2.jotform.com/submit.php" enctype="multipart/form-data" accept-charset="utf-8">

If you have the same form ID in your v2 account, You will need to login to that version so that you can get all the submissions that you received.

Please do post a reply if you need further clarification regarding this.

If you have other questions or concerns, please don't hesitate to start a new topic.

    Thanks,
