Blank Jotforms

Hello,

If you look at our SHBC Appointment Request form and SHCC Appointment Request form, the top 2 and 4 entries respectively are blank. If we download them as PDF's we can see them, but when we click on view they are blank.

I tried switching from the new version to the old version and back again and it's still not working. We can view them in the old version.

We are sorry for the inconvenience this may have caused. We have had an issue with our database system that displayed the submissions blank. Our backend team has already fixed the issue and the submissions should be displaying normally now. Please check again and let us know if you still experience any issue. We will be happy to take a look again.
