Answers to questions showing up in email but not pdf attachment

    Good afternoon. In this jot form https://form.jotform.com/203425295297057 when they fill out that they have both a case manager and therapist...the answers to the therapist and case manager questions show up in the recipient email (see picture 1) but in the pdf attachment attached with the email....it only shows the answers to the therapist questions (see picture 2). Thx!

    The problem seems to lie with the answers to the case manager questions. Because when I indicate I just have a case manager...the answers to the case manager questions appear in the email (see picture 1) but not in the pdf attachment that comes with the email (see picture 2)1624995107_60db75237bb23_1624995110_60db75261fc8b_

    Can you please delete the current PDF document and create a new one applying the desired field inputs into it?

    Then attach the new PDF document to the email notification: https://www.jotform.com/help/503-how-to-include-submissions-as-pdf-attachment-in-the-email-notifications.
