My form shows up one way on my website but when I receive the results it shows up another way. All the fields are different and some are missing. PLEASE HELP!! The Screen shot shows the submission on the right and the result on the left.Page URL:

We apologize for this inconvenience. Upon investigating your form, I have found out that there are multiple fields that are not present from your form http://www.jotform.us/form/31645093777160.
Here are the fields that are missing:
Are you a previous Customer?
From the looks of it, it seems that there were some major changes you did on the form which forces the old Questions to go blank like for instance, "Are you a previous Customer?" which was renamed to "Are you Court ordered?"
While, Town and County got replaced by "Address Field" in which it will normally warned you that if you deleted the field it will also deleted any previous data submission on a message box. In this case, the submission data attached those deleted fields on your form will be lost and no way to recover it.
I also noticed your Email Notification is not updated on the form is not updated.
You can update this by deleting the Notification using the "Delete E-mail" button found on the image above. Then follow our guide on Setting Up Email Notifications
After creating the new Notification, you should clone the form and embed using IFrame then replace the one existing from your website. This should let you correctly update the whole form with the correct future submission data. As for the lost submission data, there is no way to retrieve them.
