Please cancel my account and refund my payment

    Dear Customer Service Rep:

    When I signed up for Jotform annual plan, I was told that you had a 30 day money back guarantee. After working with Jotform almost every day, I know that it is not the proct for my needs.

    Please refund my $174 payment to my credit card ASAP.
