My Forms been disabled and i cannot enable it

Hi, i was adding an automated email with my own bank details on so that my fitness clients can pay me, im guessing this is not allowed and my form has now been disabled even thoug it says enabled.

this form is vital to my business, can you please re enable it, even if this means deleting the automatic email.
