HELLO I AM HAVING AN ISSUE WITH NOT RECEIVING THE EMAIL AND OTHER INFORMATION FOR MY EPS FORM, ALL I AM GETTING IS THEIR NAME. PLEASE HELP ASAP

Upon checking your form's notification alert, I did not find any issue with that. You should be receiving the submission emails with complete submission data.
This happens when you edit your form's notification, it becomes dirty. That means after editing the notification alert, any fields/question you add in your form will not be added automatically in your notification and needs to be added manually.
I added my own email address in your form's email alert and sent a test submission. The submission email I received had complete submission data:
