Cannot Access the Forms I Created

Greetings,

I created two forms under my username, FAJR FLAIR. The forms are below:

Inquiry Form: https://form.jotform.com/FAJR_FLAIR/mitw-speaker-inquiry-form

Speaker Confirmation Form: https://form.jotform.com/FAJR_FLAIR/MITW_Speaker_Confirmation

When I click on both of the forms, it takes me to a login page. I do not want people to have to login/create an account to fill out the form. Is there a way for people to get direct access to fill out the form?

Also, I forgot my password and clicked on "forgot password" but I have not received an email with a password reset prompt. It is not in my spam folder.
