Account Suspended

My account has been suspended. I believe it is because of a form that I created. Can you delete the form and unsuspend my account? This is my second request. Thanks

I see that your form contains fields that are collecting credit card information. Please understand that this is a violation of our Terms of Use.

I see that you have already asked us to delete the offending form. At this time I have deleted and purged the form from your account and removed the suspension.
