Error Message at the End of Form (JLC 2021-22 Registration)

When I attempt to test my form "JLC 2021-22 Registration," I am seeing an error message at the very end of the form that is prohibiting me from submitting an entry. The message says, "there are errors on this form. Please fix before continuing." I'm not sure what the issue is, and I don't know how to fix it. Please advise. Thank you.
