THUNDER INITIAL FORM. I have tested it, but as of this week each time it is filled out and submitted, it does not record as submitted. No matter the device used.

Howdy artistic12,
I have created a clone of the form and sent a test submission. It appears that the submission was recorded as it was entered on my end. I apologize for any inconvenience but could you please provide more information about exactly what is not correct in your submission records? What values have you entered, and what is being recorded?

I have replied to your emails and created a new ticket..... I URGENTLY need help!!!
Any form with a SKIP FIELD applied will not show on my end, even though it says (by checkmark) submitted on user end. Urgent since these collect important data that cannot be re-collected like hours, maintenance, etc...

I tried submitting your form and I got the following error.
The skip condition you are using in your form does help you to skip to the end of the form. However, it does not change the required field status. For this purpose, you'll have to hide the fields conditionally by adding a similar condition to hide all the cards and moving to the last card.
You can also disable the Skip condition and only use the Show/Hide condition instead.
Here is how a clone of your form is submitting after adding the above show/hide condition.
