Hello, How can I build out a quiz type form? I believe this website was built with jotform, let me know if I am mistaken.

Hey Sweet Nick,
I Understand that you want to build a quiz type form. When you say "quiz type" i believe you are referring to the one page design of the form.
This form seems to be made with the new Jotform Card form design, which shows a single question pr. page. https://www.jotform.com/cards/
Let me show you how to do this.
When creating a new form with the Card design, you can choose either to create a new from scratch or use a template.
If you decide to use a template, you can filter the results to only show templates with this new Card design by choosing Card under Filter & Sort.
If you want to start from scratch, you will immediately get the option to select this design.
From here it just works like any other form, except that each field you insert will be on a separate page.
You can also create conditional logic like on the regular form, ex. presenting different questions depending on how the form is answered.
