Struggling to create a form that generates a new report every time it is submitted

    Like the title says. I would like to create a form that when it is submitted will create a brand new report document (preferably a PDF) using the information given in the form. How do I achieve this? My goal is to ultimately have a form that has yes/no boxes. Depending on the way these have been ticked it will create a PDF document which has text on it that depends on the answers, I would also like to use Zapier to save this pdf in my sharepoint.

    This is very possible by simply creating a PDF document in PDF Editor and associating it to your form. Submissions will apply into that document automatically.

    As per integrating that PDF document to SharePoint, I'm afraid this is not possible. However, this is possible using other services such as Google Drive, Dropbox, OneDrive, etc.

    Hi I'm still confused and the video you sent me did not help me achieve what I want to. I need the submit button on a form to create a brand new pdf document every time it is submitted and fill out that pdf with information on the form.

    Hi there,
    I understand that you want to create a different PDF depending on the Yes/No answer on the form.

    To achieve that please follow these steps:

    1 . Open the PDF editor-> Create two different PDFs

    2 . Create two autoresponder emails, attach the first PDF to the first email and the second PDF to the second email

    3 . Using conditional logic you can create a rule to send the email with the correct PDF based on user's answer.
