zapier connection NOT reliably working

    Hi there - I have a zap setup that is triggered by a new form submission, but over the last 5 days it hasn't always run when there's a new form submission1626063184 60ebc1506e27e Screen Shot 202 Screenshot 10

    We appreciate you showing us a detailed explanation of the issue, in order for us to begin to replicate the issue, we would like to know the ff:

    > Were there any recent changes made to the form(added fields, changed field labels, deleted fields, etc.). If so, then it breaks the integration, you may have to re-integrate it back or, clone the form, integrate and make a test submission.

    > Is the form embedded on a website? If so, can you share the website link so we can check?
