Drivers License and Experience are not showing...

It looks like your email notification didn't update when you had updated your form which sometimes can happen. The easiest way to fix it is delete your Email Notification and then make a new one which will update your Notification with all of the correct matching fields where you can then Edit it further.
The other way is to manually do this by adding in a row where the Question would go then click on the " Form Fields " in the box on the right. Those are connected directly to the answer inputs on your form so that they carry over into your email. So for your Drivers License and Experience Fields you would need to find that and click it to generate the associated Tag. If you need further help then please provide us with a link to your form.
