I am currently in the process of creating a website and think JotForm might be able to save me hours of work.
My site allows administrators to provide feedback on users submissions.
I need a form where a user can submit basic info about themselves and upload a word or PDF document. This then needs to go to an online data base. (I can do this myself if JotForm is unable)
The evaluator then needs to be able to see this data, and enter into another form their rating of the submission (1-10 scale in 10 different catagories). I would like a visually appealing report to be generated based on the data the evaluator provides. Preferably horizontal bars from 1-10.I would like this report to be viewable both as a website and as a PDF document that could also be e-mailed to the user. (Doesn't have to be automatic)
Is this possible? I would be happy to pay extra to have someone from your team help.
Currently using wordpress for the website.

I think that's doable. Just create a form in the form builder, add an upload field if you'd like to ask user uploads.
Unfortunately we do not have built it integration for external databases but we have a workaround for that: Form to MySQL
We also have visual report if you'd like to generate graphical reports like bars or pie charts out of your form submissions. How-to-create-a-Visual-Reports-with-your-Form-Submissions
PDF Submissions is also possible if this is what you mean, please check this visual guide.
Setup & Embed > Email Alerts > Open Notification and click PDF Link to add a PDF Link variable on your email body. The generated PDF will display the data submitted by the user.
