Trying to recreate Excel Form

Hi - I am trying to recreate an excel form which has auto-calculations in it for a points program we have. Are you able to help me re-create it in JotForms? I can send you the excel file to look at to see if doable.

I can't figure out how to auto trigger for the person to fill out the same information for date & description of activity for each activity when someone selects a frequency of activity that's higher than 1. Also can't figure out how to get a calculation of points for each activity. Please help!
