In trying to fix the color on my introduction page - i messed up the header on my actual form - is t here a way to copy the intro header to my form? if not please tell me step by step how to rebuild it (I developed it months ago and i have no clue how i did it now.
I am very close to completeing this - y'all have been great.

I am sorry, but I am a bit lost here. What is the intro header, and where exactly would you like to place it?
I want the top of both my pages (forms) to look alike. i printed and scanned what i am trying to reproduce, however i can't attach it to this form. I know you all hate this but i can send in another request if that is better?
