You can use one of our Signature widgets to get this done.

We support our in-house Signature widget, as well as Adobe Sign and DocuSign.

If you'd like to fill out the form first yourself, and then only have the user sign it, you can fill it out but leave the signature empty, then send them the Edit Link so they can sign it.
