    On the challenge page...

    <iframe id="JotFormIFrame" onload="window.parent.scrollTo(0,0)" allowtransparency="true" src="" frameborder="0" style="width:100%; height:457px; border:none;" scrolling="no"></iframe>

    <script type="text/javascript">window.handleIFrameMessage = function(e) {var args =":");var iframe = document.getElementById("JotFormIFrame");if (!iframe)return;switch (args[0]) {case "scrollIntoView":iframe.scrollIntoView();break;case "setHeight" = args[1] + "px";break;}};if (window.addEventListener) {window.addEventListener("message", handleIFrameMessage, false);} else if (window.attachEvent) {window.attachEvent("onmessage", handleIFrameMessage);}</script>

    How to I get it back?

    Your form was disabled because you have exceeded your monthly quota limit. The counter will reset every 1st of the month. I can also see tha you have used the SSL instead of NON SSL take note that form Free users there are only 10 SSL submissions allowed to avoid your form using the SSL make sure to replace https://secure to http://form.

    By the way you can create a FREE account using this link as I can see that you are still using your guest account.
