Good day all.
I'm using one form to populate form two via an autoresponder email in form one. It works however when I select the values in the multiple choice checkbox filed in form one (Service(s) Provided) the same vales in form two are not selected.
My last test in form one I checked all five values. When I opened form two from the link only three of the values were checked.
I cannot figure out what is off in form one autoresponder URL to fix the problem.

Greetings Fred.
Please note that values from both fields need to match exactly.
On the first form you have Personal Care Assistant and RN Supervision, and on the second form have Personal Care Aide and RN Supervisor. This is why these two values were not filling automatically.
Please correct the value from one of the forms and let us know if the problem is resolved.

Sorry for the delayed reply.
I took what was provided and applied it to a production form. The only issue I'm having is pre-populating the Daily Rate single choice checkbox. I made some changes, tried a few things but still cannot get it to populate. Can you let me know what I'm missing/doing wrong?

Sorry if I indicated the wrong field. The field that is not populating is Rate Type.
This is what was submitted from Form 1.
Here is the population link received in the email for Form 2.
When I open the link Rate Type is not populated.

Looks like the URL is not properly configured for this field. You currently have:
rateType2={rateType1:0},{rateType1:1}
in your link. Please try to replace it with:
rateType2={rateType1}
This should resolve your problem.

That seams to work.
Just to confirm something. If a field is marked PHI it's data cannot be used to prepopulate another form, correct.
I haven't tried this yet. Can you use data from one form to populate data in a PHI field in another form?
I'm assuming you can.
